The optical stabiliser in a lens detects if there is movement using shake detecting gyro sensors that measure the angle and speed of the movement and send this information to a computer chip in the camera. This processes the information and moves a series of lens elements in the appropriate direction to counter-act the movement that the camera and thus producing a blur free photo.

So buying a digital camera with image stabilisation is very useful, they are generally more expensive but will produce better quality digital photos in almost all conditions (especially shaky ones!).
